Is it just me, or did the name of Bianca's assistant (who died in Storm Front and came back in Grave Peril as a ghost)  change between books?

Nope, it's not you. From a board discussion about evil being intent or consequences, Jim lists out several examples of the consequences side of that equation, where the intentions were good, and ends on this little tidbit:
"I just wanted to get that book finished while working two jobs and finishing a brutal semester of grad school.  It wasn't my intention to screw up the name of Bianca's personal assistant whose death had motivated her to go all power hungry to get revenge on Harry."

 in Proven Guilty, when Harry and Thomas follow the spell that Harry used to reflect the fetches back on their summoner, it takes them to the Carpenter home.  There, they see the Carpenter's van in the driveway, squashed flat-- but not long after, Charity drives the kids to St. Mary's in the family van, while Thomas and Harry follow in Madrigal Raith's rental. - Grey Paladin

Murphy has been a peace officer "since before she could drink."  Not possible; you have to be 21 to be sworn in as a police officer, or hired, or anything besides posted to the academy, at least in the state of Illinois. -Grey Paladin, who lives in Illinois and has degree in Criminal Justice,  in juvenile probation and parole

Murphy's mom didn't let her go to her own prom.  The age at which she was "allowed" to drink may not have been 21. :D
