On a more serious note- some monster just got lucky. Hell, our Harry has already nearly lost an eye, wouldn't take much to shift that event a little.

There have been a few near misses along Harry's path, but I imagine in MM his misses and apparent hit was probably from something that we would here consider an ally or some such... idk, have to examine all the places where his eye nearly gets put out..
