So after feeding some sleep to my crankyness, I think I've got something that might work rather universal.
Take out whoever you want to teleport, then make the teleportation part of the taken out result. For best results, include a symbolic link for the destination.
So if you want to teleport yourself, that's at most a 5 shift spell to take you out. I would not make the symbolic link for the target count against the complexity, that's extra. And, of course, you'd have to account for wards, if you want to burst through them, otherwise you'll be stopped rather rudely when crashing into it. If you want to take more people with you, just make the spell a zone attack for +2, or cover multiple zones if you want to take that many people. Though those +2 should probably not come from "I concentrate real hard" type declarations, but more along the lines of "I take an hour to draw runes and markings on the ground to cover everyone with my spell.

It is an interesting question though: how did those actors commit that action from your examples Hick. Spoiler time: was it actually tearing a whole in the fabric of Reality between places? Was it a manipulation of the reality of the NeverNever instead? Or is it simply a case of "because we can" by a pair of god-like beings?
Thoughts?
Ok, lets dissect those things.
First, lets look at the Dresden Files.
Harry is attempting to rescue his daughter from the red court. He knows, that he is not only outclassed and outnumbered, but also really outnumbered, really really outnumbered, and really really really outnumbered. Things don't look good.
Over the course of the story, he gets his powerful friends to help, countering the outnumbered aspect. He takes up the mantle of the winter knight, to cancel out the outclassed aspect. Then he recruits Eb, the grey council, Lea and Vadderung to help him, each aiding in canceling out the aspects stacked against him. He basically "creates" the portals by beating the beings creating them in a social conflict.
Now the Leananshide Files.
If I remember correctly, Lea basically transports the grey council within an enchanted emerald. Narratively, I think she did something similar to a shapeshifter turning into a smaller animal, she shifted most of the transported mass into the nevernever (it's in the sidebar on page 70-something, where they talk about the wereforms). The pocket in the nevernever is linked to the emerald, so she can recall everything inside the pocket any time she wants, as long as she has the emerald. Given that she also has a lot of power in the nevernever, she could have the "demesne" power and just used part of her personal nevernever real-estate. A 3-4 shift maneuver should do the trick, if you can actually change reality to do your bidding, his is nothing more than a parlor trick.
The McCoy Files.
Eb is bringing in the... birdpeople? I forgot how they were called, but he opened a portal and they poured out of it. I think this can be done like the spell I've described above, only to last longer and work better, it is sort of done as a ward. The idea would be, to create a ward that will "attack" anyone touching it with an attack powerful enough to take them out. Since it is done for volunteers, you can ignore anything and go for 5 shifts again, the volunteering should work as the catch for any powers, because it would be ridiculous to make it more complicated to teleport someone, just because they are a bit tougher. Ok, so we have the ward, people run into it and are disintegrated by the "attack". Next, we need a destination. That would be an enchanted item that will simply put the aspect "exit" wherever you put it, and that will activate the prepared ward as well, creating a portal between there and here. Since Eb planned to call in the troops, he's had lots of time preparing this spell, so he can put a lot of shifts into keeping it open long enough.
The Vadderung Files.
I think Vadderung's portal sort of works like a Stargate. You enter the gate on one side, are turned into energy and travel literally at lightning speed to the other side of the gate. Mechanically, this will sort of be like Eb's version, only with the difference, that the exit is prepared and he is creating the entrance.
Since this is happening in the aftermath, I'd be a bit lenient with the rules here and have it just stay open as long as I like. It's the magical version of the ambulances arriving at the scene of the big action battle just before the credits start rolling and they pan out over the burning ruins of the big bads lair.
