Anyone have tips on how to get a piece of writing to flow from one sentence to another, and a good way to tie together chapters?

Practice. And reading things out loud.

Or as one writer says - "Get black on white!"

i have the same issue, having my chapters flow into one another is difficult but like everyone else has said practice makes perfect. take some creative writing classes if you can it really helps.

Also, do bear in mind that control of not having smooth flow is itself a technique; sometimes what you want is to give the reader a jolt, to keep them from thinking they're sure they know what's coming next. GRRM is very good at this indeed.
